Patients are sometimes discharged from the hospital, or other facility, while still needing close medical supervision. Other times, a doctor may decide that a patient needs assistance but may not need to come to the hospital. This is where Boone Hospital Home Care steps in. Our caregivers are able to schedule home visits 24 hours a day, seven days a week with around-the-clock, on-call nursing supervision. Boone Hospital Home Care professionals also provide in-depth patient and caregiver materials, patient assessments and insurance verification assistance./ppBoone Hospital Home Care provides home care and hospice services to patients 24 hours per day, seven days per week. Home care visits are provided for patients with a skilled medical need and are intermittent in nature. Hospice services are provided for patients diagnosed with a terminal illness. Services include care from the following disciplines: nursing, therapy, aide, social work, chaplain and dietician. In addition to skilled medical care Boone Hospital Home Care professionals provide in-depth patient and caregiver education, patient assessments and insurance verification assistance. Participates in planning, organizing, and implementing the clinical orientation and staff development programs./phrpstrongResponsibilities/strong/pliAssists managers in data collection and report preparation relative to quality improvement processes, education, and outcome monitoring and benchmarking./liliFacilitates in-services and training programs for clinical and support staff relative to clinical competency and quality improvement initiatives./liliAssists with general orientation for Home Care Services new hires, trains CPR classes and fit testing, and mandatory competencies./liliEducates staff about regulatory compliance with federal, state and Joint Commission regulations/liliResponds to Additional Development Request (ADR) and requests for records/liliBJC has determined this is a safety-sensitive position.
