The primary purpose of your job position is to perform selected
acts for the promotion of health and in the care of persons who are
ill, injured or experiencing alterations in the normal health
process in accordance with the facility mission philosophy and as
directed by the Director of Operations. The Registered Nurse is
responsible for determining the nursing problems for an assigned
group of patients/resident, making plans to sole them, initiating
the plan or assigning others to implement it, and evaluating the
extent to which the plan was effective in the problem resolution.
As the Registered Nurse, you are delegated the administrative
authority, responsibility, and accountability necessary for
carrying out your assigned duties.

Major Responsibilities
- Completes the nursing assessments/MDS at the time of admission
and updates the MDS quarterly, annually and when there has been a
significant resident change.
- Identifies active resident problems requiring nursing
intervention at the time of admission and ongoing. Develops a
comprehensive plan of care for identified nursing problems.
Develops appropriate outcome criteria for each identified nursing
problem. Evaluates the effectiveness of the nursing interventions.
Evaluates the attainment of outcome criteria ongoing and at
discharge.
- Implements or delegates aspects of care in a timely manner,
including the execution of physician's orders.
- Revise the plan of care as indicated by changes in resident's
status.
- Supervises personnel with respect to accountability for
performance including hiring, training, discipline, counseling
performance evaluations, grievance process, staffing levels, and
other related personnel functions in order to improve service
levels.
- Encourages attending physicians in the treatment plan reviews,
records and signs orders and progress notes in accordance with
established facility guidelines.
- Charts nurses' notes in an informative and descriptive manner
that reflects the care provided to the resident, as well as the
resident's response to the care.
- Charts all accidents and incidents involving residents.
Performs other routine charting duties as required and in
accordance with established charting and documentation
guidelines.
- Ensures that an adequate supply of floor stock medications,
supplies and equipment is on hand to meet the nursing needs of the
residents.
- Documents all nursing care delivered to the resident. Delegates
certain certain documentation to other personnel consistent with
their job description.
- Updates the resident's medical records at the time of transfer
and discharge. Initiates and updates the Resident Profiles and
plans of care according to guidelines.
- Plans and directs nursing activities consistent with define
guidelines and standards of care. The focus of care is resident
wellness and adaptation.
- Participates in discharge planning, including communicating
nursing activities to those responsible for the resident's care
after discharge.
- Performs and monitors the appropriate transfer of residents
including the use of transfer equipment.
- Communicates care issues to other nursing staff and the
Director of Nursing.
- Assures that the nursing component of the resident record is
complete.
- Performs other duties as assigned.
Knowledge Skills and Abilities
- Knowledge of professional nursing theory and
practices.
- Knowledge of basic human physical and psychosocial
needs.
- Knowledge of human resources management including hiring
practices, disciplinary measures, termination, etc.
- Ability to apply nursing techniques to specific patient
situations.
- Ability to assign and coordinate the work of health
professionals in order to provide comprehensive care for a group of
patients.
- Ability to prepare and maintain records, charts and
reports.
- Ability to follow detailed written and oral
directions.
- Ability to read, write, speak, and understand the English
language.
- Ability to make independent decisions when circumstances
warrant such action.
- Ability to relate information concerning a resident's
condition.
- Ability not to pose a direct threat to the health or safety of
other individuals in the work place.
- Skill in the use of the sight/hearing senses or use of
prosthetics that will enable these senses to function adequately so
that the requirements of the position can be full met.
- Ability to meet the general health requirements set forth by
the guidelines of the facility, which include a medical and
physical examination.
- Ability to push, pull, move and/or lift a minimum of 25 pounds
to a minimum height of three feet and be able to push, pull, move,
and/or carry such weight a minimum distance of three feet.
Exposure Level
It is highly likely that while performing the essential functions
of theis position you will be exposed to blood or body fluids.
Minimum Qualifications
Graduate from an accredited school of professional nursing.
Licensed as a Registered Nurse in accordance with the requirements
of the state governing the facility.
One year of experience as a nurse.
